The first step in the healthcare planning program is an accurate and complete assessment. Once the first assessment is complete, a list of problems should be generated. This can be as simple as a medical diagnosis list. The list of problems can be family or relationship problems that affect the parent’s overall well-being.

After the list of problems, the caregiver must ask: “Can I solve this problem? If so, the health professional’s goal should be to solve that specific problem, and this goal should be specific, measurable and feasible. To achieve this goal must also be measurable and realistic. In the event that a medical problem is irreversible, such as diabetes, the next step would be to eliminate other complications or a possible deterioration of health. In the case of such health problems, the goal should be to keep the level of health at an optimal level.

In case of illness, where other health complications are inevitable, the goal must be to improve the quality of life. It should be noted that for all medical problems, the approach must be ordered by the doctor. The planning process for healthcare is never completed until the patient is out of the current healthcare facility. A periodic re-evaluation of the schedule is also required when the patient is out. Ultimately, the goal of the health care plan is to guide all those involved in the care of the patient and to offer appropriate treatment.
